There has been a growing number of men who are looking for procedures to enhance their “masculinity.” There are several techniques available in the market for enhancing the size of the penis. One popular example is through autologous (one’s own) fat injection. Below, Dr Roberto Viel, with over 20 years experience in penoplasty discusses in detail about the penis enlargement procedure.

WHAT IS PENIS ENLARGEMENT?

Penis enlargement is a surgical male enhancing procedure using the patient’s own fat to increase the girth of the penis.  Usually, at the same time, the patient also asks to get a lengthening procedure too.  The lengthening procedure involves the cutting of a ligament that will lengthen the penis during its flaccid state but not during erection.

WHAT TO EXPECT BEFORE THE SURGERY?

You will be instructed to avoid taking Aspirin, Vitamin E, and a large amount of garlic and ginger one week before the surgery. Avoid smoking for 48 hours before the surgery. You will be given a prescription for a medication pill which helps lessen the bruises and the swelling. This needs to be taken a week before the surgery and continued for another week after the surgery.

On the day of the procedure, you need to be fasting from midnight.

Dr Roberto Viel and our anaesthetist will see you before we start the procedure and during this time, you can address any issues you may have.

Once ready for the surgery, the anaesthetist will place a cannula in one of your hands and through this cannula, a cocktail of medications is given to start the sedation.

WHAT TO EXPECT DURING THE PENIS ENLARGEMENT SURGERY?

With intravenous sedation, you will be relaxed and will sleep through the entire procedure.

Penoplasty starts off by harvesting fat from areas where you don’t need them (e.g abdomen and flanks). This is an advantage because you can also have small liposuction in areas where you have excess fat.

Once the fat has been collected, we filter them through a special filtering technique. After filtration, the fat is ready to be injected. Firstly, the surgeon will inject local anaesthesia at the base of your penis, and introduce the fat using a small cannula. Once the maximum amount of fat has been injected, a small stitch will be placed to close the incision (surgical scar). The incision scar is quite small and will be barely visible once healed.

WHAT IS INTRAVENOUS SEDATION?

Intravenous sedation is a type of anaesthesia where patients are relaxed and are not aware of their surroundings. These sedative medications are injected through the vein. During the procedure, you will be breathing on your own and will only be given supplemental oxygen through a nasal cannula. (Unlike general anaesthesia, where the breathing capacity is compromised and a tube is required to help with breathing.) The advantage of intravenous sedation is the recovery time.

WHAT HAPPENS IMMEDIATELY AFTER THE PROCEDURE?

You are advised to stay in the clinic for approximately 2-4 hours. This can be longer depending on your recovery.

You will be placed on a support garment on the areas where the fat has been removed (e.g abdominal binder for the abdomen/ flanks). This binder is to help lessen the swelling and promote healing. You will be advised to wear the garment until the swelling subsides which is around 7 days. A dressing will be placed on the surgical scar where the fat injection has been done. 

HOW IS THE RECOVERY?

Immediately after the procedure, expect minimal pain. In case the pain worsens, this can be managed by taking pain killers. Also, the penis will also be bruised and swollen which can last for a week.

After the first week, you can resume your normal activities and as tolerated.

HOW LONG DOES IT LAST?

Technically, 20-30% of the total fat injected will be absorbed and the remaining 70-80% will remain.

It is also important to note that a few factors may also contribute to the loss of fat (eg. smoking, significant weight loss, excessive exercise). It is advised to stop smoking and maintain a constant weight.

DO I NEED MORE FAT INJECTED?

Subsequent reinjections are needed in order to achieve the expected outcome required. You need to come back for a follow-up after a month to assess your recovery.
