What is the MACS Face-Lift?

The MACS (Minimal Access Cranial Suspension) Face-lift or better known as the short-scar facelift is a simple and effective face-lift operation that is designed to lift the lower two-thirds of the face in a relatively straight forward procedure with minimal inconvenience to the patient.

 

Why is it not the same traditional ‘wind-tunnel’ face-lifts like Joan Rivers?

This is a more gentle technique lifting the face vertically, thereby, not changing much of the original structure of the face.  The old traditional methods still pull backwards, which tugs on the mouth and the corner of nose creating the unwanted ‘wind-tunnel’ effect.

What can I do to maintain this look?

Often with patients whose skin in lose on the face, they also perform the blepharoplasty (eyelid surgery) that makes the eye look heavy.  This can be done at the same time as the face-lift, giving the face an overall refreshing look.  We also recommend doing regular botox treatments to prevent wrinkles in certain areas, but we maintain that the patient should look natural at the same time, and not frozen.

How long does the MACS-Lift procedure take?

The face-lift using the MACS-Lift technique takes around 2 hours under sedation, which is not the same as general anaesthesia.  It is a mid-point between local and general anaesthesia, and yet the patient will have no memory of the procedure, known as a ‘twilight sedation’.

How long will I take to recover from the face-lift using the MACS-Lift method?

You have to wear a headband for 4 days, and the stitches are removed after 7 – 10 days.  The scars after a while are almost invisible as most of it remains with the hairline so it is a perfect procedure even if you have short hair.

Botox

Why is botox so popular during this time?

Everyone has been rushing around to finish off deadlines, do Christmas shopping and Christmas dinner preparations that they have had no time to prepare themselves.  So the quickest treatment to take away some lines and erase the woes of 2011 would be to use botox.  It takes 1 0 – 15 minutes, and there is no downtime.  The results for botox are quick and effective.  Having been used since the 1980s, it still remains one of the most popular treatments on the market these days.

How long does botox last for?

Botox should last on average 3 – 4 months in your first year.  As your facial muscles get progressively weaker, the next treatment can be repeated after 5- 6 months in the second year.

Fillers

What are fillers made from?

Non-permanent fillers should be made from hyaluronic acid.  Be careful what your doctor injects.  Always ask if you are not sure.  Any other substances could lead to many complications.  Brands such as Juvaderm and Radiesse are very well known on the market.

What are they used for?

As we age, we lose volume on the lips and upper part of the face, and this can used to make rejuvenate the contours of the face.  Also, if you have eye bags, the fillers can be used to blend in the eye bag taking years off your face.

How long does it last for?

These fillers should lasts between 8 months – 1 year.

Fancy having your answers answered by expert cosmetic surgeons?  Our readers have written to us with the following questions.  Dr Roberto Viel, from our London, Harley Street branch gives advice on a monthly basis.

Cosmetic  surgeon, Dr Roberto Viel, takes some time out of his busy schedule to answer some of your questions about fillers, mole removals, pigmentation from the sun, ageing hands.

Q. I’ve got some some lines around my mouth which make me look older than my age, what can I do to make them less visible? 

A: These lines are usually exacerbated from smoking so if you are a smoker, stop immediately.  The easier treatment option is inject a temporary filler which will last 6 months to 1 year, and botox will not work for these lines unlike other lines on the face.  The other option is more invasive treatment to wipe out the lines via a laser treatment or chemical peel, however, there is a downtime of 2 weeks with possible side-effects.  After the treatment, make sure the skin is moisturized and a total sunblock should be added to the daily face routine.

Q: I have noticed a few unsightly moles on my back, and I can’t bear wearing dresses or tops that reveal them.  Is there anything I can do about them or is it better to leave them?

A: It is possible to see a plastic surgeon to remove the moles, and the scar will be very minimal.  The surgeon will inject some local anaesthetic in the surrounding area, and the mole will be gently cut out, and very fine stitches are used to minimize the scar.  Upon request, the mole samples can be sent to a laboratory to get tested to ensure there are no abnormalities.

Q:I am becoming more and more conscious of the appearance of my veiny hands – like Madonna’s which I feel are a tell-tale sign of my age.

A:There are two main options.  As we age, we start losing fat from the hands enhancing appearance of the bones and veins making it unsightly.  A temporary solution is to use fillers to inject into the hands to give a more even and plumper appearance.  A permanent solution is to use your own (autologous) fat, which can be harvested from the abdomen or other preferred area.  This is gently and precisely removed via liposuction, and then reinjected into the hands.  These procedures can be performed under sedation or local anaesthetic.

Q:I have lived in Dubai for many years but I have moved back to London, UK, and I think that the Dubai sun has contributed dramatically to the freckles and ages spots on my face.  Is there some sort of peel I could do there isn’t too aggressive?

A: There are three options.  Firstly, there is a topical cream applied by the surgeons as a facemask, followed by a home care treatment for 2 months.  Second option, when there is a more severe condition, a fraxel laser will help with the age spots and pigmentations.  Finally, a medium to deep peel can be used to eradicate the age spots and freckles.  However, with the peel you have to be extremely careful living in Dubai to avoid the sun completely post-treatment as they can lead to hyper-pigmentation.  If you are prone to sun spots and freckles, it is important to add a strong sunblock to your facial regime.

Liposuction – 3- 6 months before wedding day

Loosing weight without surgery

Think about what you drink:
Adequate hydration will detox our entire body and enables us to burn fat faster. Abstain from alcohol, sodas, coffee and energy drinks. Instead drink at least 1.5 litres of pure water spread throughout the day to see a dramatic improvement in your skin and energy levels.

Cut out all or most sugar:
Sugar in any form is totally toxic to our entire system, not only leading to inevitable weight gain and fat storage but also causing inflammation, unbalancing blood sugar, disrupting hormone and metabolic levels as well as ageing skin cells and damaging collagen. Eliminating sugar completely is the best thing you can do for your overall health and wellbeing. Watch out for hidden sources in processed foods and always check the ingredients of everything you buy.

Choose your carbs carefully:
You don’t need to cut carbs completely, instead focus on the carbohydrate quality – think “low GL” rather than “low carb”. GL refers to glycemic load which measures the glycemic effect of foods on the body. Replace white and refined carbohydrates with 100% wholegrains and low sugar fruits and vegetables which will maintain blood sugar levels and avoid storing energy as fat.

Pack in that protein:
Getting enough good quality protein is an absolutely crucial component for any weight loss regime or healthy eating plan. The amino acids contained in protein are the building blocks of all cells and the ideal fuel to fire up your fat burning metabolic furnace. Include a portion of protein at everytime you eat.

Find the right fats:
Getting the right mix of natural fats from healthy sources (olive oil, coconut oil, oily fish, whole eggs, avocado, nuts and seeds) are essential for hormone production, boosting your metabolism and giving a healthy sheen to your hair and glow to your skin.

Fillers

Again, like botox we recommend doing a trial run.  It is important to use non-permanent fillers because permanent fillers can have serious complications after 2 – 3 years.  This can be added to the lips to give a more kissable effect but make sure this is done by a good surgeon with a good eye because the last thing that you want on your wedding day is the trout pout.  Also, you can ask a good surgeon to use the filler to disguise your eyebags.  The cosmetic surgeon is able to inject it below the bag in certain cases to blend the heavy eye bags making you look less tired and more refreshed on your wedding day.

PRP – Platelet Rich Plasma Facial Rejuvenation – an extreme but organic facial

1.  Don’t be tempted by yo-yo diets as this slows down your metabolic rate increasing your weight in the longer term.  If you want to have a good body on your wedding day you can be on a sensible diet but you MUST incorporate this with 20 minutes of exercise a day if you can.

2. Try to do exercises that incorporate the use of your core muscles.  You can even do some ballroom lessons for your big day for your first dance; do pilates at the gym or swim.  These will strengthen your core creating a flatter tummy avoiding the need to do liposuction.

It is tough being a mum of two kids physically, and whilst I don’t have any regrets about having my children, my body is not what it used to be.  I decided to start Googling plastic surgeons in London, and I came across these Italian twin plastic surgeons who have been practising for more than twenty years in London as well as a few other surgeons.

The consultation – breast augmentation with implants

It was really hard admitting to myself that I needed cosmetic surgery.  In my twenties, I swore that I would never do any plastic surgery, then my first child was born, and I watched my body bounce back into shape after about 9 months, but my breasts had lost some volume but I was still ok with them.  Then, my baby boy was born 2 years laters, and he was a little guzzler, always hungry, and I breastfed him for 6 months.  My boobs, in my opinion were a disaster and certainly not what they used to be.  The top part of my breasts had hollowed out and they look more droopy – scarily they reminded me of my mother’s breasts.  I didn’t feel great in clothes, dreaded bikinis or any figure-hugging tops.

I booked a few consultations, and I saw several surgeons before coming across Dr Roberto Viel on Harley Street, London.  I liked him because he was very approachable, and he explained the procedure to me, the risks and the benefits in leyman terms.  Most important for me was that he had over 20 years of experience and that experience for me came over during the consultation.  Compared to the other surgeons, he listened to what I wanted. I told him that I only wanted to go up a cup size to look natural and he agreed.  I was originally a C cup, and after my two children, I went back to B cup, and I just wanted to restore my youthful breasts.  During my research, I found that Dr Roberto’s best quality as a cosmetic surgeon was that he could make breast augmentations look extremely natural, and that for me was crucial.   He was able to tell me why he was going to place my implants on top of the muscle rather than under the muscle; he explained carefully where he would place the incision; and he explained what type of implants he will use – silicon in my case.  He told me that he would use a high-tech silcon implant made with adhesive silicon so if in theory you sliced the implant in half the silicon would remain in its casing.  I had mentally signed on the dotted line.

I came home and told my husband my crazy idea for my birthday present, and he was not overly thrilled with the idea but eventually agreed to the whole thing.

Surgery Day

I was a little nervous about the procedure, and the night before I couldn’t sleep thinking that maybe the whole idea was a little silly.  I had two young kids, and what was the point in taking such risks.  However, before I knew it I was at the surgery, and the anaesthetist was talking through the sedation.  My husband sat with me also as the nurse explained all the post-operative care – I guess that was his department.  I was thinking of milking the recovery for all it was worth.  Then, they told me they were ready for me in theatre.  Within minutes of being on the operating table, I assume I was asleep within seconds.

It almost felt like a good 8 hours sleep – actually I was told by Dr Roberto that it was just 1hour – I felt a little sleepy from the sedation.  I opened my eyes, spoke a little with the nurses – mouth was very dry from the sedation, but I fell back to sleep for another hour.  My husband was back in the clinic by the time I woke up again, and he looked a little stressed and tired.  He later explained that he was a little nervous, and was relieved that I was ok – he told me his imagination was starting to go a little wild in the coffee shop just round the corner.  A few hours later, I was ready to go home.  I couldn’t believe that I was on the other side – I had done my long awaited boobjob.  Finally.  After days of contemplation, and worrying and decided on the surgery, that was all over now.  I just had to wait for the results.

Because of the anaesthesia I felt no pain for the first couple of hours after the surgery, and I headed home to bed, and slept.  It wasn’t a particularly comfortable first night sleep because I was half-excited, and half worried that I was going to crush these new breasts.  Then for the next few days they were tender and sore – it felt similar to the feeling when your breasts are first engorged with milk after birth.

2 Week later

The unveiling took place 2 week later back at the clinic, and I had the stitches removed, but I was still forced to wear a sports bra, day and night for a further 2 weeks.  It was more uncomfortable then the recovery of the surgery.  The breast augmentation surgery itself was fine  - the breasts were tender but overall manageable.  I was a little worried that they were looking  a bit more perky than I had bargained for, but Dr Roberto assured me that it was fine, and it would fall and resume a more natural contour.

1 month later

Well, my husband is delighted.

He actually thanked Dr Roberto profusely – I had never seen him look so enthusiastic about anything.  I think he seemed more thankful than me, and muttered why we hadn’t thought about this earlier.  My breasts looked even better than pre-babies.  They had shape, size, and I think I have even better boobs than any 21 year girl.  What a thrill.  I am honestly so pleased and relieved with the whole experience.

3 months later

I am expecting baby number 3 unexpectedly.

Dr Roberto Viel, who is based in our 15 Harley St, London clinic, comments in the Daily Mail about facelifts and how to look natural without creating the unwanted ‘wind tunnel’ effect.  He has over 20 years experience in the business with his identical twin brother, Dr Maurizio Viel.

We also ask Dr Roberto Viel further questions about the procedure.

Q: How do you perform face-lifts, and not look like ‘Joan Rivers’?

A: For a face-lift to look natural it is important for the surgeon to follow what gravity and nature have done to the face, and simply to reverse the effect. It sounds simple but many surgeons are bent on changing the face, and the traditional face-lift surgeons still pull the face back stretching the mouth and other features creating that unwanted ‘wind-tunnel’ effect.  As we age, the face, neck skin and muscles head south, and as they droop we also loose volume on the face.  The latter is key to keeping the face young.  We vertically then lift the skin and muscles, and at the same time, we can add fillers or fat to the cheek area to re-volumise the face creating more youthful contours.

Q: When is it time for a face-lift?

A:  There are a number of factors including genetics, lifestyle and environment that have to be considered.  Asians tend to require face-lifts late in life because of their good genes, whilst Caucasians have much thinner skin on average.  The latter group’s lifestyle also weighs in heavily depending on sun exposure, drinking and smoking.  On average, our face-lift patients tend to be between 40 – 65 years old.

Q: Is it painful?

A: The pain is minimal with some swelling and bruising, and can be easily managed with some mild painkillers.

Q: How long is the recovery?

A: The patient will have an elastic garment for 4 days and the stitches can be removed after 10 days.

Q: When can I go back to normal social activities?

A: Patients usually resume their activities after 2 weeks with a refreshed look.  Most of our patients’ friends and family ask our patients if they have been on holiday, and are none-the-wiser.

Q: What is the recovery time compared to the conventional anaesthesia?  We understand that sedation is not the same as general anaesthesia, please can you elaborate?

A: Sedation is more gentle to the body and has a quick recovery time, and more often than not patients have less nausea from sedation.  Some patients who opt for general anaesthesia may want to stay overnight, whilst patient who choose sedation go home the same day.  Our face-lifts are done under sedation.

Q: What are the side effects of intravenous sedation, if any?

A: Often, the patients feel a little drowsy for a couple of hours after surgery.  This wears off after 3 – 4 hours, and patients are advised never to drive for 24 hours after surgery.  Only 10% of patients may experience some nausea.

Tired of getting up and putting on make-up every morning? If you are 
a perfectionist it might be challenging to get your eyebrows the right 
shape and shade and time is too precious, and with summer approaching 
we are spending more time at the beach and outside, and most make-up 
melts under the Middle East sunshine. We still want some of our make- 
up to enhance our face, but not have it run down our faces. No amount 
of waterproof make-up can last the afternoon at the beach. We speak 
to Candice Watson, renowned semi-permanent make-up artist to tell us 
more about her work. She has over 20 years experience and has worked 
on London’s prestigious Harley Street.

Many of us are afraid of tattooing our eyebrows or do eyeliner because 
we have seen many of the older generation with blue eyebrows. Why is 
your method so different?

The blue was from the use of permanent make-up, and the traditional 
ink has a blue base causing the blue eyebrow look. In the past, they 
had to inject it deeper into the skin making the treatment extremely 
painful. Today, most highly skilled artists should only use semi- 
permanent inks that use natural based mineral pigments. This is 
placed in the dermal layer, the outermost layer of the skin so with 
the skin’s natural monthly exfoliation process it will gradually 
disappear. Unfortunately, in the Middle East, we have seen many 
artists stick with traditional inks which are cheaper, and the results 
are not as superior.

There appears to be confusion about the regulation of the semi- 
permanent make-up industry? If you are looking for semi

permanent make up Dubai, how do we know that we are going to 
someone qualified and safe?

When people are choosing a technician for their semi-permanent make-up 
they should find out what experience the technician has, where they 
have worked before, make sure they are using a digital machine and not 
a rotary one and check that the pigments are not permanent ones. They 
should also ask to see previous work that the technician has done from 
her portfolio and to speak to previous patients.

Many technicians in the Middle East have not been correctly trained 
and do not seem to adhere to any health and safety procedures, and as 
a result I have had to do many removal treatments on clients with the 
blue and pink eyebrows and purple lips! I now work closely with the 
Dubai Municipality to set down the correct guidelines and regulations 
for Dubai, I am basing these on the guidelines from my days on Harley 
Street, UK. Please also check that all needles used are used once only 
and then thrown away! I have heard stories here that people are re- 
using needles or giving the needle to the client for when they come 
back for their top up and then using the needle again for the top up, 
this is unacceptable and will cause infections or worse.
